 FREE HEALTH MANAGMENT PROGRAM

The Stanford Chronic Disease Self-Management Program (CDSMP) is a six-week workshop conducted in two and a half hour sessions in community settings. The program was developed and tested by Stanford University to help people learn the techniques and strategies they need for the day-to-day management of chronic disease. In Michigan, CDSMP is known as PATH, which stands for P ersonal A ction T oward H ealth.

PATH is designed to benefit adults with chronic or long-term health conditions including arthritis, heart disease, diabetes, emphysema, asthma, bronchitis and depression. Family members, friends, and caregivers are also encouraged to attend the workshop.
Program participants reported improved symptom management, communication with physicians, self-efficacy, physical activity, and general health compared to those who had not participated in the workshop. Additional studies showed that program participants spent fewer nights in the hospital and used fewer health services over a two-year period compared to their status prior to the program, which yielded a cost-to-savings ratio of approximately 1:10. Many of these results persisted for as long as three years .

The program is called “Riding Toward A Successful Future,” and will be the featured message on STARS buses.

“Our goal is to increase access to summer recreation for kids,” said Cherrie Benchley, president of United Way of Saginaw County. “We want kids to have a place to go this summer that offers them constructive fun activities in a safe environment.”

Youths ages 11 to 18 can ride a STARS bus for free to and from recreation programs at the following organizations: Boys & Girls Club at Merrill Park, Longfellow School and Stone School; First Ward Community Center, including the Civitan and Cathedral District program; Lake Huron Area Boy Scouts; Neighborhood House; Salvation Army Summer Day Camp and the YMCA.

The special initiative is being funded by a grant from the H.R. Wickes Foundation.

Benchley said students interest in participating can register with one of the listed recreation programs and receive a bus pass. To ride the bus, they must show the drive the bus pass and a picture ID, providing them transportation to and from the event at no cost.

“To expand access to more programming and events, staff from the partnering summer recreation programs will be able to take children younger than age 11 on field trips, or share events with other organizations,” Benchley said.
